tegra2: Add Avionic Design Medcom support.

The Medcom is a 16:9 15" terminal that is used for patient infotainment
in hospitals.

Changes in v3:
  * Remove unused implementation of gpio_config_uart().
  * Implement MMC/SD card detection.
  * Drop board_mmc_getcd() which is now implemented by common Tegra2
    code.
  * Add MAINTAINERS entry.

Changes in v2:
  * No longer override the default CONFIG_SYS_TEXT_BASE setting.
